Hyde Primary Care Network shortlisted for Nursing Times Award

CLEAR dementia transformation project at Hyde Primary Care Network (PCN) shortlisted for Nursing Times Award after delivering £1 million in NHS savings and cutting hospital admissions by 27%.

An innovative dementia case management service developed by Healthy Hyde PCN as part of the CLEAR Proactive Care Programme has been shortlisted for a 2025 Nursing Times Award in the Nursing in Primary Care category, recognising its outstanding contribution to patient care and nurse-led service transformation. 

The service, designed and led by nurses from the PCN with support from the CLEAR Programme, has transformed outcomes for people living with dementia in the community and boosted efficiency. In its first year, the service achieved a 27% reduction in emergency hospital admissions among its dementia cohort, and within two years, delivered over £1 million in hospital cost savings.  

Additionally, the PCN saved around £160,000 - equivalent to eight additional GP sessions per week—one per practice. 

The project emerged through the National CLEAR Programme’s Proactive Care initiative, which supported and empowered local NHS teams to develop proactive, data-driven care models for people at high risk of health deterioration. By using the CLEAR methodology, the Hyde team identified gaps in dementia care, particularly among people living at home, and designed a holistic service that delivers coordinated support through specialist nurse case managers and case navigators.

This shortlisting is richly deserved.”

The Nursing in Primary Care category recognises nurse-led projects that deliver innovative, measurable improvements in patient care and service effectiveness. It celebrates leadership and impact in managing long-term conditions and providing essential frontline care, highlighting initiatives that can be shared and adopted across primary care. 

The Nursing Times Awards celebrate the very best in nursing excellence, innovation, and leadership across the UK. The official shortlist was announced today, 26 June 2025, and the winners will be revealed at a high-profile awards ceremony on Wednesday 22 October 2025 at the JW Marriott Grosvenor House Hotel on Park Lane, London.
